What we need from you:

        1.  Patient Information Sheet

        2.  Both sides of patient's insurance card(s)

  There are three ways to get information from your office to us:

        1.  Faxing (available 24 hours a day)

        2.  U.S. Mail

        3.  Pickup by us (available to local clients)

Checks still go to your office. You need only fax us a copy of the EOB (Statement received with the checks) and we will post payments to patient accounts in order to provide you with monthly reports and to thoroughly collect what you are owed.
